FOR: Girls 13 to 18 years old.
DATE: January 16th & 19th 2012
TIME: 3:30 - 5:30 PM
PLACE: The Gabe Nesbitt Park of McKinney 7001 W. Eldorado Pkwy. McKinney, TX. 75070
BRING: CHANT forms listed below.
Also, Glove, Cleats, Bat, Helmet, & Face Mask if desired (Mandatory for pitchers/catchers)
EXPECTATIONS of ATHLETES: Catch Without Dropping; Throwing Using Proper Technique; Run Bases, Slide, & Dive Head 1st; Hitting & Bunting; Know Basic Position Responsibilities-Including While at Bat.
Parents Required To Print & Complete the Listed Forms From the CHANT Forms Page
1. Copy of a 2012 (06/2011 & after is acceptable) Sport’s Physical, Performed, Dated & Signed by: Dr. of M.D.; Physician’s Assistant; Nurse Practitioner; or Dr. of Chiropractic. Designated ‘Clear’ for Activities. ONLY the 2nd page of the physical form is required for CHANT.

3. CHANT Family Registration Form & $10 Annual Membership Dues
4. CHANT Softball Registration Form
5. CHANT Code of Conduct Agreement Form (For Brand New CHANT Members Only – not last season’s players)
