From Aug. 29th – End of season. The last practice day will be before the last scheduled game, more details to come later.

Please remember to send in all the forms to the CHANT office by 8/15/11. All of the forms requested for football registration are on the CHANT Forms page and can be printed from there.
1. CHANT Family Reg. ($10 annual fee for all participants in CHANT)
2. CHANT Football Reg. (Team fees and reg. payment deadline are listed on form)
3. CHANT Release of Liability Waiver (If not previously turned in for 2011)
4. CHANT Conduct Agreement (Read, sign, and return only the bottom slip portion, you keep the top portion)
5. Copy of your child’s physical for 2011, performed and signed off as being ‘clear’ to play in sport’s related activities by either licensed: Physician, Physician’s Assistant, Nurse Practitioner, or Dr. of Chiropractic.
